Instructions for using the web-based version
Currently using old schema
- Go to the website: Forms4UsAll Online
- Sign In:
- Type in a user name or create a new account
- If you enter invalid username, you will be prompted to create a new account
- If name is taken, you will be prompted for a new entry
- Pick a form from the list of "Available Forms"
- Filling out the Form:
- If the right panel does not reflect the same form as the left, click on refresh to refresh the page
- Fill in the fields in the left frame of the page. Click on "CHANGE" to see the changes you've made reflected in the right frame
- You can continue modifying the form until you've clicked on "SUBMIT". This submission will update your user information in our database.
- The right frame shows the actual form you are filling out. DO NOT CLICK ON "SUBMIT" IN THIS FRAME

- On Submission:
- If the form is submittable, the actual form will be displayed, filled out, in the left frame. If all fields are correct, clicking on "SUBMIT" will actually submit the form.
